SWAP, the School to Work Alliance Program, is a part of the Colorado Division of Vocational Rehabilitation and the Cherry Creek School District. (SWAP/ DVR) helps young adult ages 17-25, with disabilities prepare for, obtain, keep, or regain suitable jobs. If eligible, SWAP/ DVR services will be provided to meet your individual employment needs, and you are expected to work at the end of SWAP/ DVR services.
To be eligible for DVR services you must:
1. Have a valid state issued ID and a social security card. Please site Resources for information on how to obtain these documents.
2. Have a documented mild to moderate disability which creates a barrier to employment for you, and
3. Need vocational rehabilitation (VR) services from SWAP/ DVR to achieve employment.
4. Intend to achieve an independent employment outcome.
SWAP/ DVR will need personal information to determine your eligibility. If you are eligible, this information will also help you develop your Individualized Plan for Employment and carry it through until you have a job. SWAP will stay involved for at least the first 90 days of employment and connected until one year after those 90 days. Any information we receive will be held confidential. Failure to follow through during these services could show cause for closure from the program.
SWAP was created by a collaboration of DVR and the Colorado Department of Education. If you are ready and able to work hard, invest the time, and demonstrate good work ethics, contact your school staff or SWAP/ DVR to apply.
